Job Summary: Serves in an expanded nursing role to collaborate with high risk beneficiaries and their promary care provider and care team to provide a model of care that ensures the delivery of quality, efficient and cost effective health care services. Assesses, plans and implements all options and services with the goal of optimizing the beneficiaries health status. Integrates evidence based clinical guidelines in the development of care plans.

 

Job Responsibilities:

 

  • Effectively utilizes risk stratified data to identify targeted population .
  • Assesses the health, education and psychosocial needs of the targeted benificiaries and designs an individualized plan of care.
  • Fosters a team approach by working effectively with the beneficiary, family members, health care team to ensure coordination of services.
  • Implements clinical interventions based on risk stratification and evidence based clinical guidelines; collaborates with the health team, to enhance evidence based guideline adherence and promote best practice(s) by initiating and adjusting plan a care as directed by the provider and providing appropriate follow up and monitoring as needed.
  • Coordinates referrals and community services.
  • Identifies gaps in care and appropriately intervenes
  • Accurately documents all interactions in a timely manner.
  • Effectively incorporates written, verbal and listening skills into practice. Utilizes motivational interviewing techniques and teach back methodology.

 

  • Continuously evaluates utilization (IP, ED) and quality metrics for assigned population to monitor effectiveness.

 

  • Maintains up to date knowledge regarding effective engagement strategies, care management approaches and behavior change strategies into clinical practice.

 

  • Works collaboratively with inpatient and SNF care managment teams to foster effective transitions of care.

 

  • iWorks closely with Care management team members and payer partners  to develop disease management and population health initiatives to meet the goals of the orgnaization and payer partners.

 

  • Demonstrate professional, appropriate, effective and tactful written, verbal and nonverbal communication with patients, families, medical staff, colleagues and other deportments and entities throughout the continuum of care to promote continuity of care and high level services to the patient population.

 

Requirements:

 

  • Valid PA RN license required
  • Previous care management experience preferred
  • Previous Ambulatory/Medical Practice experience preferred
  • 2 years relevant experience required
  • Valid driver’s license required
