Job Summary: Collaborates with all disciplines for the purpose of assessing, planning, implementing and

evaluating/documenting psychosocial factors which contribute to patients' illnesses and/or have an impact on continuity of care; scope of services may include, but is not limited to, discharge planning, counseling, crisis intervention, information and referral, domestic violence, drug and alcohol abuse, suspected abuse, psychiatric services and guardianships per regulatory requirements. Services may be provided both inpatient and outpatient.

Job Responsibilities:

  • Completes and documents psychosocial assessments, intervention, and coordination of care to identified populations. (25%)
  • Provides crisis intervention/counseling to identified populations to enhance patient/family coping mechanisms regarding illness. (25%)
  • Engages the patient/family/significant other and interdisciplinary team in the discharge process to identify appropriate post acute resources and community service follow-ups. (25%)
  • Provides education to patients/family/significant other and interdisciplinary team in regard to psychosocial factors related to illness. (15%)
  • Actively participates in those areas of expertise to further enhance the role of social work within the hospital. (10%)
  • Completes duties as required for the purpose of discharge planning, i.e. options forms, Kane, MA51, agency referrels, etc. Complies with standards and requirements in medical/legal reporting situations. Complies withsocial work competencies by department and hospital standards. Communicates telephonically andelectronically with the outpatient providers in an effort to enhance the continuum of care.
  • Assumes responsibility for AHN required continued education and own professional growth.
  • Performs other duties as assigned or required.

Education: MSW required for new hires. Attainment of MSW within five (5) years of hire date required as condition ofemployment. Current employees grandfathered

Licensure/Certification/Registration: LSW or LCSW eligibility preferred depending upon facility requirements. Older Adults Protective Services Act (OAPSA) clearance may be required depending on location.

Complexity of Work: Requires interpersonal, analytic and critical thinking skills for problem solving and decision making. Decisive judgment and the ability to work with minimal supervision. Must be able to work in a fast paced environment requiring adaptability, flexibility and independence while demonstrating high professional standards, leadership and team membership skills.

Personal Protective Equipment: Standard Precautions as required for specific task.

Required Experience: 2 years experience in a hospital/multidisciplinary health care setting preferred.
