Job Summary: Coordinates the clerical and administrative processes of utilization management and discharge arrangements. Completes any other tasks within department guidelines requested by the Nurse Navigator and Quality Nurse
Job Responsibilities:
- Assists with heart failure certification processby identifying patients for post discharge calls and follow up appointments to be completed; follows up with third party payers as needed; assists in the acquisition of authorizations for post-acute services;
- Supports discharge plan implementation under direction of the care coordinator, nurse navigator and social worker, which may include: faxing to payers/post-acute providers; calling vendors, physician offices, clinics, etc.; arranging for coordinating and scheduling home health needs; identifying community resources; completing appropriate discharge documentation; identifying primary care physicians and scheduling pcp/specialty appointments as needed; updating patient demographic/billing data; assisting with placement process.
- Collects/enters data as directed and performs other administrative duties as needed.
- Adheres to the policies, procedures, rules, regulations, and laws of the hospital and all federal and state regulatory bodies.
Education: High school degree or equivalent
Complexity of Work: Requires problem solving skills although most information is well-defined and minimal clarification is necessary. Specific projects and goals are assigned with latitude for decision making and setting of priorities. Supervision is readily available and work may be checked for accuracy. Ability to work with minimal supervision. Requires interpersonal skills, communication skills (both personal and telephonic), and the ability to work without supervision when necessary.
Required Experience: Three (3) years of relevant experience, preferably in a healthcare or service environment Proficient computer skills with experience in: Microsoft office, internet navigation, clinically- based software, and financially-based software. Knowledge of medical terminology required.
